You will be emailed initial paperwork and consent documents after you provide your email address through our secure site.  This will speed up the Intake process and you can save these documents in the patient portal.

You will need to upload your driver's license or picture ID into the secure portal prior to the initial appointment.  

For children with a custody arrangement between two caregivers, a copy of the custody legal paperwork or divorce decree is required to upload to the secure portal, or you can scan and email the document to counseling@aileenwainwrightlcsw.com or FAX them to 819-379-1933.

This is to ensure that proper legal consent is given and protocol is followed.

If you wish to provide a copy of any recent past Psychological Evaluations or discharge instructions, or your child's school IEP, Special Education or 504 documents if applicable, these can sometimes be very helpful additions in formulating a comprehensive treatment plan.
