Do you take insurance and how does payment work?

  

This office is currently in-network with the following insurance companies:
   ​​

  • Blue Cross / Blue Shield PPO  and Federal Plans

  • Cigna

  • Medicare

  • United Behavioral Health  (OPTUM)

 

Please Contact your insurance provider for your specific behavioral health benefits prior to scheduling your initial appointment to verify your deductible, co-insurance, co-pays and any out of pocket expenses.

***THIS OFFICE IS NOT AUTOMATICALLY A PROVIDER FOR EMPLOYEE ASSISTANCE (EAP) BENEFITS FOR ANY INSURANCE COMPANY OR EMPLOYEE PROGRAM.

A Superbill will be emailed to self-pay clients  or clients paying out of pocket with "out of network benefits" who wish to submit to other insurance companies for reimbursement (although full or partial reimbursement is not guaranteed). 

 

This office accepts payment via debit, VISA Mastercard, American Express and Flex-Spending account Cards for payment and insurance co-payment.  Due to Telehealth counseling only - I require that all clients keep on file a credit card or Flex spending card in the portal.
